Person-centred practice involves listening to people and supporting them to think about what they want from their lives now and in the future. It involves thinking outside the square to figure out how to support aspirations and goals as well as supporting the person to make informed choices and understand risks and responsibilities.

A person-centred plan is a description of the things we have learned and discovered about the person. It includes things the person wants to do now and in the future, how they would like to be supported to achieve these things, who they would like to be supported by and what meaningful support means to them.
A person-centred plan takes a person’s whole of life into account. It is a living document and needs to be reviewed and built on as the person’s wants, needs and experiences change. It includes an up to date profile of the person which details their strengths and skills, what is important to the person and what staff need to know to best support them. It also includes a detailed action plan to address what needs to be done to ensure that the person is supported to achieve his or her goals and aspirations.
The plan is developed with the contribution of those that know the person best and those that the person choses to be involved in their planning. This may include the person’s family members and friends, work colleagues, paid and non-paid professionals and other services. These people form the person’s circle of support and will come together regularly for the person’s planning meeting to develop the plan.

Active support is a proven model that focuses on each person’s engagement and contribution. It uses different support and communication techniques to enable successful participation.
This means that people are encouraged to be part of all activities throughout the day and in all aspects of their own life. They are supported to contribute their skills, the things they can do, and to make informed decisions and choices.
The underlying principles of active support involve supporting meaningful activities and relationships, helping people to gain more control over their own lives and to become valued members of their community.
