‘My Home’

Supported Living

Drop-in support – Independent Living or In-home Support – ‘Support to live in my home’

We can drop-in to your own home to provide support, whether you live independently or with your family. We can arrange times and a schedule that suits you. Your program is developed for you and you may want to learn new skills, access the community or need a little help to maintain your social life.

Group Homes – ‘long term support’

In Northern Sydney we provide over 20 group home services in the community where residents live their life in a way that is no different to others in the community. Our accommodation facilities provide a safe and supportive home environment for people with a range of needs.

All placements are made through ADHC, please click here to contact ADHC.

Community Support Program – ‘Help me in my home’

The Community Support Program offers extra support to clients who wish to remain in their own home, but need some support to achieve this. Types of skills that we can help you to develop include daily personal tasks such as showering, getting dressed and making a meal. Sunnyfield provides a reliable, flexible service that meets your needs. We also encourage our clients to be involved with the selection of their own staff.

Self-Managed Services

Self-Managed Services – ‘My choice my way’

We can build a program around you, based on your skills and interests. We partner with community organisations to make sure you get to do more of the activities you enjoy.
Tom’s story is an example of what we can achieve together, click here to see his story.

Covers funding model: Individualised Accommodation Support Package (IASP), Supported Living Fund (SLF), Self-Managed Community Participation, Life Choices and Active Ageing

Northern Beaches – Behaviour Intervention Service (NS-BIS)

Sunnyfield’s Behaviour Support Services are funded to provide the NS-BIS service for children and young people on the Northern Beaches. This service supports families to respond to behaviours and emphasises a positive and supportive approach to care.

Northern Sydney Respite– Behaviour Support Implementation Service

Sunnyfield works alongside respite centres, families and Behaviour Clinicians to implement Behaviour Support Plans for children and adults accessing respite accommodation in the Northern Sydney Local Planning Area (LPA). Our service enables you to continue to access respite services and increases the capability of support workers in all aspects of behaviour support.

Fee for Service

Sunnyfield is able to provide additional support services on a fee for service basis. This means that we can provide a service that is outside of a government funding package, or in excess of your allocation.  This may mean someone to provide additional support or more time in a program such as our day options programs.

Please contact us for more information and to discuss the ways in which a fee for service arrangement may benefit you.
