Being Person Centred – What it really means...
Sunnyfield encourages and supports people with an intellectual disability to follow their dreams and gain greater independence – whether that involves making new friends, learning new things, trying a sport or hobby or participating in the community. The people we support are encouraged through their network of support to make decisions about what they want in their lives and how they want their lives to look into the future.
We do not do ‘initial assessments’ as many other organisations may do, rather we assist the person we are supporting to themselves identify what their goals and dreams are and then helping them match their goals and dreams with their own capabilities for now and into the future. Outcomes are tailored to meet with the support that is available and to then ‘facilitate’ future supports to help meet the eventual goal and aspiration of that person.
Sunnyfield's person centred vision also impacts on every department and every person in our organisation. More specifically Sunnyfield has Person Centred Coordinators who support the operational divisions by assisting key staff in the development of person centred plans and practices.
